Qualifications

• Basic Life Support (BLS) certification from American Heart Association; must obtain within 90 days of hire.
• Excellent customer service and telephone communication skills; proficient with computer software; able to handle confidential information.

Responsibilities

• Participates in daily huddles to determine patient flow and gaps in care.
• Acts as clinic EHR super user and provides training for new team members.
• Coordinates scheduling, referrals, health records, and data retrieval from automated systems.
• Assists in empanelment by assigning patients to a primary care provider and scheduling accordingly.
• Maintains provider clinic schedules, tracks patient panels, and ensures appointments with the PCP.
