Duly Health and Care in Naperville, IL is seeking a Patient Access Conversion Specialist for a full-time, onsite role from 8:30 AM to 5:00 PM, Monday through Friday. You will be part of a physician-led team focused on delivering high-quality, coordinated care in a patient-centered environment.

Responsibilities include outbound calls, scheduling and transportation support, greeting patients, EPIC data verification, and monitoring daily appointment reports to optimize slot utilization.

Qualifications

• High school diploma or GED required.
• Three years of patient/client-facing experience preferred.

Responsibilities

• Make high volume outbound calls to fulfill patient needs over the phone.

• Verify patient information and document in EPIC.
• Review schedules and monitor appointment reports.
• Pre-screen patients per departmental protocol and ensure slots are filled.
• Monitor daily scheduling reports and prepare charts as needed.
