Job summary

Florida Health Care Plans seeks a Patient Navigation Manager to optimize access to care across FHCP clinics. You will own patient workflow experiences and drive process improvements with a focus on reliability, collaboration, and data-informed decisions.

The role requires strong leadership, excellent communication, and the ability to manage multiple tasks in a fast-paced healthcare environment. Monday–Friday 8 am–5 pm, with 1st shift expectations and readiness for emergency situations.

Qualifications

• Bachelor's degree preferred or 3 years management/supervisory experience required.
• Ability to form strong teams that produce measurable results.
• Experience with complying with regulations for managed care organizations, preferred.
• Strong interpersonal and collaboration skills across stakeholders.
• Demonstration of process improvement experience.
• Experience using data analytics tools to identify risks and support issues.
• Strong verbal, written, analytical and interpersonal skills.
• Ability to organize, prioritize work and meet deadlines.
• Proficient computer skills in EPIC, AS400, Health Trio, Availity and Outlook.
• Knowledge of current Patient Service and Experience trends and best practices.
• Capable of working independently and in multi-disciplinary teams.
• Flexible schedule between 7am-7pm and readiness for emergencies.
