Job summary

HCA Florida West Tampa Hospital is seeking an experienced Patient Care Navigator RN to guide patients through the healthcare system, coordinating access and support services with compassion. In this role you will serve as a single point of contact for referrals, help patients understand diagnoses and treatment options, and collaborate with the care team to address physical, social and emotional needs.

Qualifications

• Associate degree in health care field; Bachelor’s degree preferred.
• Minimum 1 year clinical specialty area experience.
• Current Florida State Registered Nurse license.
• Current BLS (Basic Life Support).
• Strong critical thinking and interpersonal communication; proficient reading and PC skills.

Responsibilities

• Serve as a single point of contact for physicians, patients and caregivers to ensure access to clinical and support care services.
• Assist patients in understanding their diagnosis, treatment options, and the resources available.
• Assist patients and families in establishing a support network addressing physical, social, and emotional needs.
• Communicate effectively and work with physicians and/or other members of the interdisciplinary team in meeting patient needs.
