Job summary

Hoyleton Youth & Family Services in Illinois seeks a Pathways Youth Coordinator II to provide care coordination to youth benefiting from coordinated community services within the Pathways to Success framework. This role supports families through crisis prevention and safety planning, and helps form Child and Family Teams to access needed resources.

You will maintain records, manage a 1:16 caseload, and collaborate with providers to ensure youth needs are met.

Qualifications

  • Bachelor’s degree or Illinois Recovery Specialist with two years supervised experience.
  • Ability to work in a fast-paced, multidisciplinary team.
  • Knowledge of community programs and evidence-based practices.

Responsibilities

  • Evaluate referrals and contact family within seven days.
  • Complete strengths, needs, and cultural assessment using IM+CANS.
  • Develop and maintain Crisis Prevention and Safety Plan with youth and family.
  • Organize and run Child and Family Team meetings every 60 days.
  • Maintain timely, professional case records per guidelines.
  • Manage caseload ratio of 1:16 in the service area.
  • Coordinate linkage to in-network providers and MCO Case Manager.
  • Deliver trauma-informed, family-centered services.
