Job summary

Hoyleton Youth And Family Services seeks a Pathways Youth Coordinator II to provide care coordination for youth benefiting from community services. This role supports families to build a vision, crisis plans, and a child and family team, aligning services to youth goals within Pathways to Success.

The position emphasizes trauma-informed, family-centered care, collaboration with schools and providers, and diligent record-keeping in accordance with organizational guidelines.

Qualifications

• Bachelor’s degree in counseling, guidance, social work, education, psychology, or related field, or Illinois Certified Recovery Support Specialist.
• Two years’ supervised experience is required.
• Ability to work in a fast-paced, multidisciplinary team environment.
• Knowledge of community programs and services; familiarity with child-serving systems.

Responsibilities

• Evaluate and interpret referral information within 7 days.
• Create strengths-based service plans using IM+CANS.
• Maintain documentation in the provider portal and adjust IM+CANS as needed.
• Develop Crisis Prevention and Crisis Safety Plans with youth and family.

POSITION SUMMARY

ThePathways Youth Coordinator IIis part of the Care Coordination and Support program and provides care coordination to youth who would benefit from coordinated community services. This program was developed in response to HFS Pathways to Success.

The roles of Care Coordination and Support are to partner with families to understand their vision, help create Crisis Prevention and Safety Plans, support the development of a Child and Family Team, and provide education and linkage to resources and services that help families and youth achieve their goals.

The Pathways Youth Coordinator II is responsible for the design and delivery of services that meet the youth’s needs and provide support to youth and families who have been referred to the Intensive Care Coordination Tier of Pathways to Success.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES

• Evaluate and interpret referral information and attempt contact with family within 7 days of referral.
• Complete a strengths, needs, and cultural diversity assessment with the child and family and use the IM+CANS to create an individualized strengths-based service plan.
• Review the initial IM+CANS and update any subsequent IM+CANS as required by HFS. Upload documentation into the HFS IM+CANS Provider Portal.
• Develop, document, and maintain a Crisis Prevention and Crisis Safety Plan in conjunction with the youth, parent, guardian, or caregiver.
• Assist families in identifying natural and professional supports to create a Child and Family Team and facilitate Child and Family Team Meetings every 60 days and within 48 hours after a crisis event.
• Engage the family weekly with at least one monthly face-to-face home or community visit.
• Provide linkage to in-network providers of any recommended services and collaborate with the youth’s MCO Case Manager, as applicable.
• Deliver strength-based, family-centered, culturally competent, and trauma-informed facilitation services.
• Engage with treatment providers, schools, and other collateral contacts to ensure the youth’s needs are being met.
• Maintain case records and notes in a timely and professional manner in accordance with Care Coordination and Support Organization requirements.
• Maintain a caseload ratio of 1:16 within the designated service area.
• Complete other duties as assigned.

Requirements

QUALIFICATION REQUIREMENTS

• Strong organizational and time management skills.
• Excellent interpersonal and written communication skills.
• Must possess the knowledge, personal qualities, and practice skills to work effectively with professionals in other organizations and from other disciplines and to provide services in a manner that reflects the values of the System of Care.
• Must meet DCFS/CANTS Clearance Requirements.
• Must be at least 21 years of age.
• Must possess and maintain a valid driver’s license and maintain proof of automobile insurance.

EDUCATION AND/OR EXPERIENCE

• Bachelor’s Degree in a counseling and guidance, rehabilitation counseling, social work, education, psychology, or related human service field, or an Illinois Certified Recovery Support Specialist along with two years’ supervised experience is required.
• Demonstrated ability and willingness to actively participate in a fast-paced, multidisciplinary team environment.
• Knowledge of community programs, services, and evidence-based practices, and a variety of treatment modalities used to work with youth and their family to navigate child serving systems.

TCI (Therapeutic Crisis Intervention)

Employees must:

• Pass the written test annually
• Pass the physical test every six months (if applicable to their role)

WORKING CONDITIONS

This position operates in a [professional office / community-based environment and may require travel to agency sites / client locations / partner organizations based on role responsibilities.

The role involves periods of walking, standing, or sitting, and may occasionally require climbing stairs, bending, kneeling, or lifting materials up to 10 pounds depending on job duties.

Minimal exposure to environmental factors such as noise, dust, or varying indoor/outdoor temperatures may occur depending on program needs and client-related activities.
