Job summary

Elizabethtown Community Housing is seeking a Supportive Services Navigator to provide trauma-informed case management within the Community Resource Center. You will help individuals and families access housing resources, coordinate services, and lead the Pathways program, as well as manage seasonal Emergency Shelter cases.

The role requires strong interpersonal skills, experience with homelessness or housing instability, and excellent documentation and organizational abilities.

Qualifications

• Bachelor’s degree or equivalent experience in Human Services, Social Work, Behavioral Health, or related field.
• Experience serving individuals experiencing homelessness, housing instability, trauma, or poverty preferred.
• Strong documentation, time management, and organizational skills.
• CPR and First Aid certification preferred.

Responsibilities

• Provide direct service coverage at the Community Resource Center, including intake, screenings, and referrals.
• Manage housing resources, benefits, employment, transportation, healthcare, and other supports.
• Lead Pathways program case management, eligibility, service planning, and follow-up.
• Coordinate Emergency Shelter case management during shelter season and ensure documentation meets funder requirements.
• Maintain HMIS and other databases with timely notes and progress updates.
• Collaborate with landlords and community partners, participate in team meetings and trainings.
