Job summary

The Salvation Army is seeking a Case Manager for the Pathway of Hope program to coordinate social services, assess families, and provide direct assistance and referrals. You will create case plans, conduct home visits, and maintain thorough case records while collaborating with staff and community partners.

This full-time role emphasizes strength-based case management, housing stability, increased income, and self-determination for families with minor children, with occasional after-hours

Qualifications

• Education: Degree in human service area; bachelor’s preferred.
• Experience: 2+ years in case management in social services.
• Skills/Abilities: outreach, communication, collaboration, and crisis planning.

Responsibilities

• Outreach to community sites to identify eligibility requirements.
• Conduct screening interviews with potential applicants per POH requirements.
• Develop a case plan with participants focusing on housing stability and financial resources.
• Maintain comprehensive case files and documentation for each participant.
• Coordinate with staff and contracted services to meet participant needs.
• Provide advocacy and connect participants to community resources.
• Attend meetings and maintain relationships with community agencies.
