The Salvation Army USA Eastern Territory in Norwalk, OH is seeking a Pathway of Hope Case Manager to provide strengths-based, client-centered support for families with children over a 6–18 month period. You will assess needs, craft individualized action plans, and link families to internal and community resources to promote stability and self-sufficiency.

A bachelor’s degree in social work or a related field is required, with a master’s degree preferred.

Qualifications

• Bachelor’s degree in Social Work, Human Services, Counseling, Psychology, or a related field required; master’s degree preferred.
• Prior case management experience with families, individuals experiencing poverty, or community-based social service programs is preferred.

Responsibilities

• Identify, assess, and enroll eligible families in Pathway of Hope.
• Provide ongoing case management to a caseload of approximately 4-6 families, meeting weekly or bi-weekly for 1–2 hour sessions over 6–18 months.
• Conduct intake assessments, develop goal plans, monitor progress, and complete exit and follow-up sessions.
• Introduce pastoral care and corps-based support services to families as needed.
• Assist clients with life-skill development including job readiness, budgeting, and education.
• Connect families to Salvation Army programs and external resources (housing, healthcare, benefits, etc.).
• Maintain complete case records in Community Services database and prepare monthly reports.
