Job summary

The Salvation Army Norwalk Corps is seeking a Pathway of Hope Case Manager to provide strengths-based, client-centered case management for families with children seeking stability and self-sufficiency over a 6-18 month period.

You will assess needs, develop individualized action plans, connect families to internal and community resources, and support progress toward education, employment, housing, and financial stability while maintaining accurate records and reporting as required.

Qualifications

• Bachelor’s degree in Social Work, Human Services, Counseling, Psychology, or related field required; master’s degree preferred.
• Prior case management experience with families or poverty-related programs is preferred; LICSW or related credential is preferred.

Responsibilities

• Identify and enroll eligible families in Pathway of Hope.
• Manage a caseload of approximately 4-6 families.
• Conduct intake assessments and develop individualized goal plans.
• Monitor progress and complete exit and post-exit follow-up sessions.
• Connect families to Salvation Army programs and external resources.
• Support families with life-skill development and job readiness.
• Maintain complete case records and prepare monthly reports.
