Loving Angels Network is seeking a dedicated Support Coordinator to oversee the provision of waiver and related services. The role centers on crafting and maintaining an individualized service plan with the participant, family, and planning team to ensure person-centered outcomes.

Responsibilities include coordinating resources, monitoring service delivery within budget, and facilitating ISP and PCPT processes to keep the individual at the center of planning and service decisions.

Responsabilidades

• Coordinate community resources to ensure waiver services meet eligibility and need-based criteria.
• Access resources via natural supports and through state/local/federal programs as appropriate.
• Coordinate services between and among agencies to avoid duplication of services.
• Document planning outcomes and facilitate completion of ISP and PCPT for the participant and planning team.
• Schedule planning team meetings and distribute the ISP/PCPT to involved individuals and providers.
