Job summary

Babcock Center, Inc. is seeking a dedicated Person-Centered Plan Coordinator to develop, coordinate, and review individualized support plans (ISPs) for residents.

This role collaborates with families, guardians, and an interdisciplinary team to ensure plans reflect preferences, needs, and outcomes. The coordinator facilitates ISP meetings, maintains documentation, and promotes active participation by the person supported and their network.

Qualifications

• Bachelor's degree in Human Services or related field.
• Minimum of 1 year experience with individuals with intellectual and developmental disabilities.
• Knowledge of person-centered planning processes and residential service delivery.
• Strong written, verbal, organizational, and interpersonal skills.
• Ability to coordinate and facilitate meetings involving multiple participants.
• Ability to work independently, manage deadlines, and maintain accurate documentation.
• Ability to travel between Residential locations.

Responsibilities

• Develop, write, and update individualized support plans (ISPs) for assigned persons.
• Collaborate with the person supported, family, guardians, Residential staff, Documentation Specialist, and interdisciplinary team to develop ISP outcomes and supports.
• Coordinate and facilitate annual ISP meetings and 30-day new admission ISP meetings.
• Maintain calendars and send meeting invitations to required participants.
• Identify and prepare required team members for ISP meetings.
• Communicate with team members to ensure assessments and supporting documentation are ready for meetings.
• Facilitate ISP meetings that reflect the person’s preferences and voice.
• Maintain relationships with families/guardians and encourage participation in ISP planning.
• Assist the person supported in identifying choices and desired outcomes across life areas.
• Review and monitor ISP relevance and implementation.
• Work with Residential Planning Coordinator on training goals to incorporate into ISP.
• Participate in CTMs and other team meetings related to ISP outcomes and progress.
• Ensure ISP revisions are completed timely based on changes.
• Maintain compliant documentation and confidentiality.
