Job summary

CareMed Health Solutions is seeking a Wound Care Nurse Practitioner to deliver wound services to residents in long-term care facilities. The role emphasizes collaboration with interdisciplinary teams and adherence to quality of care standards.

The ideal candidate holds a KY NP license, board certification, and experience with wound management, including chronic and infected wounds, with familiarity in wound VACs and DME.

Qualifications

• Must hold current unrestricted KY NP license.
• Board Certification required.
• DEA licensure (optional).
• Clinical background in adult, family, or geriatric care.
• Experience with wound care and chronic, debilitating, infected wounds.
• Ability to diagnose and treat diabetic, arterial, venous, and mixed etiologies wounds.
• Familiar with wound care products and wound VACs.
• Experience in long-term care settings with EMR.
• Ability to work in an interdisciplinary care team.

Responsibilities

• Provide wound care services for long-term care patients in skilled nursing facilities.
• Prescribe medications, including controlled substances per KY law.
• Order, review, and follow up on treatments and diagnostics.
• Communicate with nursing home staff about changes in condition.
• Collaborate with the primary care physician and care plan.

We are currently seeking Wound Care Nurse Practitioners to provide wound services to patients living in long-term care facilities.

WOUND NP ROLE AND RESPONSIBILITIES:

• Provide wound care services for long-term care patients residing in skilled nursing facilities.
• Prescribe appropriate medications, including controlled substances in accordance with KY Law.
• Order, review, and/or follow-up with treatments and diagnostic testing.
• Communicate with the nursing home staff on a regular and as needed to discuss changes in condition.
• and/or the care plan.
• Collaborate with the primary care physician.

WOUND NP QUALIFICATIONS AND EDUCATION REQUIREMENTS

• Current, unrestricted NP license in the state of Kentucky.
• Board Certification.
• DEA licensure (Optional)
• Clinical background in adult, family, or geriatric specialties.
• Clinical background of wound care; including but not limited to chronic, debilitating, stalled, or infected wounds
• Must be comfortable with diagnosing and treating diabetic, arterial, venous, and mixed etiology wounds or Kennedy Ulcers.
• Must be comfortable staging pressure injuries according to the NPIAP
• Must be familiar with wound care best practice and advanced wound care products; collagens, alginates, foams, silvers, etc.
• Must have experience with wound vacs and other DME, such as pressure relieving/ reducing devices
• Strong preference to having wound certification or be open to getting certified
• Experience working in a long-term care setting.
• Ability to work with an EMR.
• Ability to work within an interdisciplinary care team to coordinate patient care
