Job summary

CareMed Health Solutions is seeking a Wound Care Nurse Practitioner to provide wound services to residents in long-term care facilities. The role focuses on advanced wound care and medical management in an interdisciplinary setting.

The NP will diagnose and treat wounds, prescribe medications including controlled substances as permitted by Kentucky law, and coordinate treatments with nursing staff and primary care physicians to support patient care plans.

Qualifications

• Current, unrestricted NP license in Kentucky.
• Board Certification as nurse practitioner.
• Wound care experience in long-term care settings.
• Experience with wound vacs and advanced wound care products.
• Familiar with diagnosing and treating diabetic, arterial, venous wounds.
• Proficient with EMR and interdisciplinary teamwork.

Responsibilities

• Provide wound care services for long-term care patients in skilled nursing facilities.
• Prescribe medications, including controlled substances in accordance with KY law.
• Order, review, and follow up with treatments and diagnostic testing.
• Communicate with nursing home staff regularly about condition changes and care plans.
• Collaborate with the primary care physician.

We are currently seeking Wound Care Nurse Practitioners to provide wound services to patients living in long-term care facilities.

WOUND NP ROLE AND RESPONSIBILITIES:

• Provide wound care services for long-term care patients residing in skilled nursing facilities.
• Prescribe appropriate medications, including controlled substances in accordance with KY Law.
• Order, review, and/or follow-up with treatments and diagnostic testing.
• Communicate with the nursing home staff on a regular and as needed to discuss changes in condition.
• and/or the care plan.
• Collaborate with the primary care physician.

WOUND NP QUALIFICATIONS AND EDUCATION REQUIREMENTS

• Current, unrestricted NP license in the state of Kentucky.
• Board Certification.
• DEA licensure (Optional)
• Clinical background in adult, family, or geriatric specialties.
• Clinical background of wound care; including but not limited to chronic, debilitating, stalled, or infected wounds
• Must be comfortable with diagnosing and treating diabetic, arterial, venous, and mixed etiology wounds or Kennedy Ulcers.
• Must be comfortable staging pressure injuries according to the NPIAP
• Must be familiar with wound care best practice and advanced wound care products; collagens, alginates, foams, silvers, etc.
• Must have experience with wound vacs and other DME, such as pressure relieving/ reducing devices
• Strong preference to having wound certification or be open to getting certified
• Experience working in a long-term care setting.
• Ability to work with an EMR.
• Ability to work within an interdisciplinary care team to coordinate patient care.
