Job summary

CareMed Health Solutions is seeking a Wound Nurse Practitioner to deliver advanced wound care services for residents of long-term care facilities in Kentucky. The role requires state-licensed NP, board certification, and experience with wound management and interdisciplinary care teams.

You will prescribe therapies, order and review treatments, collaborate with primary care physicians, and work with EMR to coordinate patient plans.

Qualifications

• Current, unrestricted NP license in Kentucky.
• Board certification required.
• DEA licensure optional.
• Clinical background in adult, family, or geriatric specialties.
• Wound care experience with chronic or infected wounds.
• Comfort diagnosing and treating diabetic, arterial, venous, and Kennedy ulcers.
• Comfort staging pressure injuries per NPIAP.
• Familiar with advanced wound care products.
• Experience with wound VACs and DME.
• Wound care certification preferred.
• Experience in long-term care setting.
• Ability to work with an EMR.
• Ability to work in an interdisciplinary team.

We are currently seeking Wound Care Nurse Practitioners to provide wound services to patients living in long-term care facilities.

WOUND NP ROLE AND RESPONSIBILITIES:

• Provide wound care services for long-term care patients residing in skilled nursing facilities.
• Prescribe appropriate medications, including controlled substances in accordance with KY Law.
• Order, review, and/or follow-up with treatments and diagnostic testing.
• Communicate with the nursing home staff on a regular and as needed to discuss changes in condition.
• and/or the care plan.
• Collaborate with the primary care physician.

WOUND NP QUALIFICATIONS AND EDUCATION REQUIREMENTS

• Current, unrestricted NP license in the state of Kentucky.
• Board Certification.
• DEA licensure (Optional)
• Clinical background in adult, family, or geriatric specialties.
• Clinical background of wound care; including but not limited to chronic, debilitating, stalled, or infected wounds
• Must be comfortable with diagnosing and treating diabetic, arterial, venous, and mixed etiology wounds or Kennedy Ulcers.
• Must be comfortable staging pressure injuries according to the NPIAP
• Must be familiar with wound care best practice and advanced wound care products; collagens, alginates, foams, silvers, etc.
• Must have experience with wound vacs and other DME, such as pressure relieving/ reducing devices
• Strong preference to having wound certification or be open to getting certified
• Experience working in a long-term care setting.
• Ability to work with an EMR.
• Ability to work within an interdisciplinary care team to coordinate patient care
