Job summary

CareMed Health Solutions is seeking a Wound Care Nurse Practitioner to deliver wound care services to residents in long-term care facilities. You will diagnose, treat, and coordinate care with an interdisciplinary team to improve residents’ quality of life.

Ideal candidates hold a Kentucky NP license, board certification, and wound-care experience with skills in wound vacs and EMR systems. DEA licensure is optional, and collaboration with primary care physicians is essential.

Qualifications

• Current, unrestricted NP license in Kentucky.
• Board certification.
• DEA licensure (Optional).
• Clinical background in adult, family, or geriatric specialties.
• Clinical background of wound care: chronic, debilitating, stalled or infected wounds.
• Comfort diagnosing and treating diabetic, arterial, venous, and mixed etiology wounds or Kennedy ulcers.
• Ability to stage pressure injuries per NPIAP.
• Familiar with wound care best practice and advanced wound care products (collagens, alginates, foams, silvers, etc.).
• Experience with wound vacs and other DME.
• Preference for wound certification or willingness to become certified.
• Experience in a long-term care setting.
• Ability to work with an EMR.
• Ability to work with an interdisciplinary care team.

Responsibilities

• Provide wound care services for long-term care patients in skilled nursing facilities.
• Prescribe medications, including controlled substances per KY law.
• Order, review, and follow up on treatments and diagnostic tests.
• Communicate with nursing home staff about changes in condition and care plan.

We are currently seeking Wound Care Nurse Practitioners to provide wound services to patients living in long-term care facilities.

WOUND NP ROLE AND RESPONSIBILITIES:

• Provide wound care services for long-term care patients residing in skilled nursing facilities.
• Prescribe appropriate medications, including controlled substances in accordance with KY Law.
• Order, review, and/or follow-up with treatments and diagnostic testing.
• Communicate with the nursing home staff on a regular and as needed to discuss changes in condition.
• and/or the care plan.
• Collaborate with the primary care physician.

Wound Np Qualifications And Education Requirements

• Current, unrestricted NP license in the state of Kentucky.
• Board Certification.
• DEA licensure (Optional)
• Clinical background in adult, family, or geriatric specialties.
• Clinical background of wound care; including but not limited to chronic, debilitating, stalled, or infected wounds
• Must be comfortable with diagnosing and treating diabetic, arterial, venous, and mixed etiology wounds or Kennedy Ulcers.
• Must be comfortable staging pressure injuries according to the NPIAP
• Must be familiar with wound care best practice and advanced wound care products; collagens, alginates, foams, silvers, etc.
• Must have experience with wound vacs and other DME, such as pressure relieving/ reducing devices
• Strong preference to having wound certification or be open to getting certified
• Experience working in a long-term care setting.
• Ability to work with an EMR.
• Ability to work within an interdisciplinary care team to coordinate patient care
