Job summary

CareMed Health Solutions in Kentucky seeks a Wound Care Nurse Practitioner to deliver specialized wound care in long-term care facilities. You will assess, diagnose, and treat complex wounds, coordinate with facility staff, and participate in care planning.

Requires current KY NP license, board certification, and familiarity with wound products and wound vacs. Part-time or full-time availability; EMR experience preferred; compensation commensurate with experience.

Qualifications

• Current, unrestricted NP license in Kentucky.
• Board Certification.
• DEA licensure (Optional).
• Clinical background in adult, family, or geriatric specialties.
• Clinical background of wound care; including chronic, debilitating, stalled, or infected wounds.
• Must be comfortable diagnosing and treating diabetic, arterial, venous, and mixed etiology wounds or Kennedy Ulcers.
• Must be familiar with wound care best practice and advanced wound care products; collagens, alginates, foams, silvers, etc.
• Must have experience with wound vacs and other DME, such as pressure relieving/reducing devices.
• Strong preference to having wound certification or be open to getting certified.
• Experience working in a long-term care setting.
• Ability to work with an EMR.
• Ability to work within an interdisciplinary care team to coordinate patient care.

Responsibilities

• Provide wound care services for long-term care patients residing in skilled nursing facilities.
• Prescribe appropriate medications, including controlled substances in accordance with KY Law.
• Order, review, and/or follow-up with treatments and diagnostic testing.
• Communicate with the nursing home staff on a regular and as needed to discuss changes in condition and the care plan.
• Collaborate with the primary care physician.
• Coordinate with an interdisciplinary care team to coordinate patient care.

WOUND NP QUALIFICATIONS AND EDUCATION REQUIREMENTS

• Clinical background of wound care; including but not limited to chronic, debilitating, stalled, or infected wounds
• Must be comfortable with diagnosing and treating diabetic, arterial, venous, and mixed etiology wounds or Kennedy Ulcers.
• Must be comfortable staging pressure injuries according to the NPIAP
• Must be familiar with wound care best practice and advanced wound care products; collagens, alginates, foams, silvers, etc.
• Must have experience with wound vacs and other DME, such as pressure relieving/ reducing devices
• Strong preference to having wound certification or be open to getting certified
• Experience working in a long-term care setting.
• Ability to work with an EMR.
• Ability to work within an interdisciplinary care team to coordinate patient care.
