Job summary

CareMed Health Solutions is seeking a part-time Wound Care Nurse Practitioner to provide wound services to residents in long-term care facilities. You will diagnose and treat wounds, prescribe medications (including controlled substances per KY law), order and follow-up with treatments and tests, and communicate changes with nursing home staff to update the care plan.

The ideal candidate has a current, unrestricted KY NP license, board certification, experience with chronic and vascular wounds,

Qualifications

• Current, unrestricted NP license in Kentucky.
• Board Certification as NP.
• Clinical background in adult, family or geriatric specialties.
• Clinical background of wound care including chronic, debilitating, stalled or infected wounds.
• Comfort diagnosing and treating diabetic, arterial, venous and mixed etiology wounds; Kennedy ulcers okay.
• Familiar with staging pressure injuries per NPIAP; wound care products; wound vacs and DME.

Responsibilities

• Provide wound care services for long-term care patients in skilled nursing facilities.
• Prescribe appropriate medications, including controlled substances per KY law.
• Order, review and follow up with treatments and diagnostic testing.
• Communicate with nursing home staff regularly about changes in condition and care plan.
• Collaborate with the primary care physician to coordinate patient care.
• Work within an interdisciplinary care team to optimize outcomes.

We are currently seeking Wound Care Nurse Practitioners to provide wound services to patients living in long-term care facilities.

WOUND NP ROLE AND RESPONSIBILITIES

• Provide wound care services for long-term care patients residing in skilled nursing facilities.
• Prescribe appropriate medications, including controlled substances in accordance with KY Law.
• Order, review, and/or follow-up with treatments and diagnostic testing.
• Communicate with the nursing home staff on a regular and as needed to discuss changes in condition.
• and/or the care plan.
• Collaborate with the primary care physician.

Wound Np Qualifications And Education Requirements

• Current, unrestricted NP license in the state of Kentucky.
• Board Certification.
• DEA licensure (Optional)
• Clinical background in adult, family, or geriatric specialties.
• Clinical background of wound care; including but not limited to chronic, debilitating, stalled, or infected wounds
• Must be comfortable with diagnosing and treating diabetic, arterial, venous, and mixed etiology wounds or Kennedy Ulcers.
• Must be comfortable staging pressure injuries according to the NPIAP
• Must be familiar with wound care best practice and advanced wound care products; collagens, alginates, foams, silvers, etc.
• Must have experience with wound vacs and other DME, such as pressure relieving/ reducing devices
• Strong preference to having wound certification or be open to getting certified
• Experience working in a long-term care setting.
• Ability to work with an EMR.
• Ability to work within an interdisciplinary care team to coordinate patient care.
