Job summary

Integrated Wound Care is seeking a compassionate Nurse Practitioner to join our team on a part-time basis. The role focuses on wound assessment, treatment planning, and collaboration with a multidisciplinary care team in skilled nursing, long-term care, and rehabilitation settings.

The ideal candidate has an active Minnesota NP license and at least 1 year of wound care experience, with strong communication, documentation, and time-management skills.

Qualifications

• Active license in state of employment (Minnesota).
• Minimum 1 year of wound care experience.
• Ability to work independently and as part of a team.
• Excellent verbal and written communication skills.
• Strong time management and organizational abilities.

Responsibilities

• Conduct wound assessments and provide personalized treatment plans for patients.
• Work collaboratively with a team of healthcare professionals to ensure optimal patient care.
• Complete documentation accurately and in a timely manner.
• Educate patients and their families on wound care, prevention, and management.
• Monitor patient progress and adjust treatment plans accordingly.
