Job summary

Reliant Rehabilitation is seeking a Speech-Language Pathologist to provide therapy in Assisted Living, Independent Living, and Memory Care settings. The role is part-time with benefits, Consistent 20-25 hours per week, and requires state licensure in speech-language pathology.

The SLP will screen, evaluate, develop treatment plans, manage discharge, supervise Rehab Techs, and ensure compliance with regulations. Travel between facilities is required in a supportive team environment.

Qualifications

• English reading, writing, and comprehension skills.

Responsibilities

• Screen residents and evaluate/treat those referred for therapy.
• Develop initial and ongoing treatment plans of care.
• Discharge planning and supervision of assistants.
• Document accurately in the medical record per guidelines.
• Collaborate with interdisciplinary team to ensure patient-centered care.
