Job summary

Reliant Rehabilitation is seeking a Speech-Language Pathologist to provide comprehensive SLP therapy and related services in skilled nursing facilities. You will screen, evaluate, and develop treatment plans, while maintaining positive interactions with facilities and clients in alignment with Reliant Rehabilitation guidelines.

Responsibilities include ongoing case management, discharge planning, and ensuring quality care with adherence to state and federal regulations.

Responsibilities

• Provide Speech-Language Pathology (SLP) therapy and related services for patients while maintaining positive interactions with facilities and clients in accordance with SLP therapy principles and Reliant Rehabilitation guidelines.
• Develop initial and ongoing treatment plans of care, discharge plans, and case management responsibilities.
• Maintain professional conduct representing Reliant Rehabilitation’s commitment to quality and service in all interactions with clients, facilities, and colleagues.
• Adhere to all state and federal regulations regarding speech-language pathology therapy.
