Job summary

Escondido Pa is seeking a Speech-Language Pathologist to join our Mesa Hills Post Acute team on a part-time PRN basis. The SLP evaluates and treats residents, coordinates with families and the care team, and maintains documentation in the medical records.

Responsibilities include developing individualized treatment plans, supervising CFYs or students where applicable, training staff, and participating in care conferences to support discharge planning and quality care.

Qualifications

• Bachelor’s degree in Speech-Language Pathology required; Master’s preferred.
• Licensed as a Speech-Language Pathologist in the state.
• Proficient with electronic medical record systems (Net Health, PCC).

Responsibilities

• Evaluate residents to determine speech therapy needs and rehab potential.
• Develop and implement individualized treatment plans.
• Provide staff training and supervise CFY/Students when applicable.
• Maintain timely, accurate documentation per regulations and facility policy.
• Participate in care conferences and discharge planning.
