Job summary

Lone Tree Post Acute in California is seeking a compassionate Speech-Language Pathologist to join our rehabilitation team on a part-time basis. The SLP will evaluate residents with speech, language, cognitive, and swallowing disorders and develop individualized treatment plans.

The ideal candidate holds a California SLP license, a Master’s in Speech-Language Pathology, ASHA CCC-SLP is preferred, and prior skilled nursing or post-acute experience is a plus. New graduates are encouraged to apply.

Qualifications

• California SLP license required.
• Master's degree in Speech-Language Pathology from an accredited program.
• ASHA CCC-SLP is preferred.
• Experience in skilled nursing or post-acute setting is a plus.

Responsibilities

• Evaluate residents for speech, language, cognitive, and swallowing disorders.
• Develop and implement individualized treatment plans.
• Provide skilled speech-language therapy based on residents’ needs.
• Monitor progress and adjust treatment plans as appropriate.
• Collaborate with nursing, therapy, and interdisciplinary team members.
• Educate residents, families, and caregivers regarding treatment recommendations.
• Maintain accurate and timely clinical documentation.
