Job summary

Sunnyvale Post Acute in Sunnyvale, CA is seeking a compassionate Speech-Language Pathologist (SLP) to join our rehabilitation team. This part-time and PRN opportunity focuses on improving residents' communication, swallowing, and overall quality of life.

Responsibilities include evaluation, individualized treatment, and collaboration with an interdisciplinary team to develop effective plans. California licensure is required, with experience in post-acute care preferred.

Qualifications

• Current California SLP license required.
• Certification and credentials to practice in California required.
• Experience in skilled nursing, post-acute care, or rehabilitation preferred.
• Strong communication, clinical, and organizational skills.
• Compassionate, resident-centered approach.
• Ability to work effectively in an interdisciplinary team.

Responsibilities

• Evaluate residents and develop individualized speech, language, cognitive, and swallowing treatment plans.
• Provide skilled therapy services based on resident needs and care plans.
• Monitor resident progress and modify treatment approaches as appropriate.
• Complete accurate and timely clinical documentation.
• Collaborate with nursing, therapy, dietary, and other interdisciplinary team members.
• Educate residents, families, and caregivers regarding treatment recommendations and strategies.
• Promote resident safety, independence, communication, and quality of life.
• Maintain compliance with professional, state, and federal standards.
