Job summary

Lone Tree Post Acute is seeking a compassionate Speech-Language Pathologist (SLP) to join our rehabilitation team on a part-time basis. This role offers a supportive environment where you can make a meaningful difference for our residents in a post-acute setting.

Requirements include a California SLP license, a Master’s degree in Speech-Language Pathology, and ASHA CCC-SLP is preferred. New graduates are encouraged to apply, with collaboration across nursing and therapy teams.

Qualifications

• Current California Speech-Language Pathologist license (SLP).
• Master’s degree in Speech-Language Pathology from an accredited program.
• ASHA CCC-SLP certification is preferred.
• Experience in a skilled nursing or post-acute setting is a plus.

Responsibilities

• Evaluate residents for speech, language, cognitive, and swallowing disorders.
• Develop and implement individualized treatment plans.
• Provide skilled speech-language therapy based on residents’ needs.
• Monitor progress and adjust treatment plans as appropriate.
• Collaborate with nursing, therapy, and interdisciplinary team members.
• Educate residents, families, and caregivers regarding treatment recommendations.
• Maintain accurate and timely clinical documentation.
