Job summary

Advocate Health's Sherman Hospital - Outpatient Rehabilitation is seeking a part-time Speech-Language Pathologist in Illinois. Requires Master's in Speech Pathology, CCC SLP, IL license; BLS within 90 days. 20 hours per week with one late shift until 7:00 p.m.

You will evaluate patients with various speech, language, voice, cognitive and swallowing disorders; establish care plans; document; discharge with home programs; collaborate with physicians and teammates. A $2,500 sign-on bonus offered.

Qualifications

• Master’s degree in Speech Pathology from an accredited school.
• CCC SLP and IL licensure required.
• BLS for Healthcare Providers within 90 days.
• Experience in outpatient settings preferred.
• Ability to work with diverse patient populations.

Responsibilities

• Interprets physician referrals and conducts speech-language evaluations.
• Implements written plan of care and delivers individualized treatment.
• Monitors outcomes and adjusts treatment plans as needed.
• Completes discharge planning and home programs; educates patients/families.
• Maintains documentation, scheduling, and billing per policy.
