You will work with the interdisciplinary team to promote quality of life.

The role involves discharge planning, referrals, and community connections; requires a Bachelor's in social work or related field and state licensure as required.

Qualifications

• Bachelor's degree in social work or human services.
• State licensure or certifications as required by state to practice in long-term care.

Responsibilities

• Collaborate with the interdisciplinary team to promote and protect patient/resident rights and well-being; educate staff on the social services role.
• Develop a written interdisciplinary plan of care to address social service needs.
• Hold meetings with patients, families, and significant others to provide support and information.
• Perform discharge planning and make referrals to appropriate agencies and suppliers.
• Establish relationships with community-based agencies and maintain referral flow.
