Job summary

Genesiscareers seeks a Social Services Specialist for our nursing center to identify social, emotional, and mental health needs of patients/residents and connect them with appropriate services, while upholding NASW ethics and regulatory standards.

You will collaborate with the interdisciplinary team to develop care plans and educate staff on social services, participate in discharge planning, and help maintain community agency referrals to enhance quality of life.

Qualifications

• Bachelor's degree in social work or human services field.
• Must possess any certifications/licensures as required by State of employment to practice in long-term care.

Responsibilities

• Collaborate with the interdisciplinary team to promote and protect patient/resident rights and well-being. This includes educating staff on the role of social services.
• Participate in the development of a written, interdisciplinary plan of care for each patient/resident to address social service needs.
• Hold meetings with patients/residents, families, and significant others as requested and to provide support and information to improve quality of life.
• Perform patient/resident discharge planning activities such as family/patient education, nursing center documentation and make referrals to appropriate agencies and suppliers.
• Establish relationships and maintain contact and referral flow with community-based agencies/services that benefit our patient/resident population.
