- Complete psychosocial assessments identifying social, emotional, and psychological needs and strengths; assess discharge planning.
- Identify discharge goals at admission and document initial discharge plan with patient/resident and family involvement.
- Educate staff on the role of Social Services and psychosocial needs of residents and families.
