Qualifications

• Bachelor's degree in social work or related field required.
• Two+ years delivering social services to elderly/family populations preferred.
• Knowledge of supportive services and local community resources.
• Demonstrated ability to advocate, organize, problem-solve, and deliver measurable results for residents.

Responsibilities

• Advocate, organize, problem-solve and deliver results for residents.
• Coordinate programs and activities for residents.
• Liaise with community agencies and service providers; seek new services for residents.
• Identify low-cost service providers and negotiate discounts.
• Engage residents to identify needs and refer to community agencies when needed.
• Develop a Resource Directory of state/local providers.
• Sponsor educational events on health care, agency support, life skills, and referrals.
• Provide quality customer service addressing concerns and assisting with basic needs.
• Assist residents accessing services like housekeeping, meals, transportation, personal services, financial assistance, daycare counseling.
• Monitor and relay feedback from residents and families.
• Manage relationships internally and externally with effective communication.
