Qualifications

• Education: Bachelor’s degree in social work or related field preferred.
• Experience: 2+ years delivering social services to elderly/family populations preferred.
• Working knowledge of supportive services and local community resources.
• Demonstrated ability to advocate, organize, problem-solve, and deliver measurable results for residents.

Responsibilities

• Advocate, organize, problem-solve to deliver results for residents.
• Coordinate programs and activities for residents.
• Liaise with community agencies and providers to obtain services for residents.
• Identify low-cost service providers and negotiate discounts.
• Engage residents to identify needs and refer to community resources.
• Develop a Resource Directory of local services.
• Sponsor educational events on health care, agency support, life skills, and referrals.
• Provide quality customer service and assist with basic needs for residents.
• Assist residents in accessing community services such as meals, transportation, etc.
• Monitor and relay feedback from residents and families.
• Manage internal and external relationships with effective communication.
