National Church Residences in Stamford, CT is seeking a Part-time Service Coordinator for 24 hours per week at Harboursite-77units. You will advocate for residents, coordinate programs, and connect them with community resources to improve living and care.

Qualifications

• Bachelor’s degree in social work or related field preferred.
• Two or more years of social services experience with elderly/family populations preferred.
• Experience advocating and coordinating services with community resources.

Responsibilities

• Advocate for residents and coordinate programs and activities.
• Serve as liaison to community agencies and service providers.
• Identify low-cost providers and negotiate discounts when possible.
• Engage residents to identify needs and make referrals to services.
• Develop a Resource Directory of local/state service providers.
• Sponsor educational events on health care, support services, and life skills.
• Provide quality customer service and address resident concerns.
• Assist residents in accessing housekeeping, meals, transportation and personal services.
