JASA, a NYC-based nonprofit, is seeking a compassionate Case Manager to support older adults and their families in community settings. This part-time role focuses on in-home assessments, service planning, and connecting clients to vital resources across NYC neighborhoods.

You will work two days per week (14 hours) at both 65 West 90th Street, Manhattan and 230 Jay Street, Brooklyn, contributing to care coordination under government funding requirements.

Competenze

• Bachelor’s degree in social work, human services, psychology, gerontology, or equivalent required.
• Experience in case management, social services, or eldercare preferred.
• Ability to travel NYC via public transit and navigate buildings is required.

Mansioni

• Conduct in-home and office-based psychosocial assessments.
• Develop, implement, and maintain individualized service plans.
• Assist with applications for Medicaid, Medicare, SNAP, and other benefits.
• Coordinate direct supportive services (homecare, transportation, housing, legal referrals).
• Evaluate cognitive and physical capacity and protect vulnerable adults as needed.
• Conduct periodic reassessments and monitor progress with supervisors.
