JASA, one of NYC's leading nonprofits serving older adults, invites you to join as a Case Manager. You will assess needs, develop care plans, and coordinate essential services to help seniors age safely in place.

Under supervision of master’s-level staff, you’ll supervise interns and home health aides, maintain precise electronic records, and advocate for clients to access benefits and supports.

Competenze

• Graduate of an accredited college or university with a BSW, BA, or BS degree (or equivalent).
• Strong writing and data organization skills; proficient with electronic records.
• English proficiency required; knowledge of additional languages is helpful.

Mansioni

• Conduct comprehensive in-home and in-office assessments of clients' social and emotional needs.
• Develop and execute written case plans; provide individual/family counseling to preserve client autonomy.
• Screen clients for entitlement eligibility, complete applications, and advocate for benefits.
• Coordinate direct services including homecare, transportation, meals on wheels, legal aid, and emergency care.
• Maintain precise electronic case records, reports, and statistics; represent JASA at meetings and conferences.
• Supervise interns, community aides, homemakers, home health aides, and volunteers.
