JASA is seeking a compassionate Case Manager to join our community services team on a part-time basis (two days per week). You will conduct in-home and office-based psychosocial assessments, build comprehensive written service plans, and connect older adults to vital community resources across NYC.

This role operates under social work standards and funder requirements, advocating for clients, coordinating with homecare, transportation, housing, and legal referrals, while maintaining accurate

Competenze

• Bachelor's degree in Social Work or related field required.
• Experience in case management or eldercare preferred.
• Ability to travel NYC via public transit and negotiate stairs.
• Bilingual proficiency in Spanish is a strong plus.
• Strong written and verbal communication; culturally competent.

Mansioni

• Conduct in-home and office-based psychosocial assessments.
• Develop, implement, and monitor individualized service plans.
• Connect clients to Medicaid, Medicare, SNAP, Home Care, Meals on Wheels, and other services.
• Coordinate direct services with homecare attendants, transportation, housing, and legal referrals.
• Maintain complete, audit-ready case notes and documentation.
• Represent JASA in community outreach and group activities.
