JASA is seeking a compassionate Case Manager for a part-time role in New York City. You will perform in-home and office-based psychosocial assessments, create comprehensive service plans, and connect older adults to vital community resources across NYC neighborhoods.

Required are a Bachelor’s in Social Work or equivalent, with preferred experience in case management or eldercare. Travel by public transit is expected; bilingual Spanish is a strong plus.

Qualifications

• Education: BSW or equivalent required.
• Experience in case management or community-based eldercare preferred.
• Ability to travel NYC via public transportation for home visits.
• Bilingual proficiency in Spanish is a strong plus.
• Proficiency with client databases and documentation.

Responsabilités

• Conduct in-home and in-office psychosocial assessments.
• Develop, implement, and maintain individualized service plans.
• Connect older adults to Medicaid, Medicare, SNAP, and other benefits.
• Coordinate homecare, transportation, housing, and legal referrals.
• Document case notes and progress in electronic systems.
