Job summary

JASA in Brooklyn is seeking a compassionate Social Worker to join our Brooklyn Case Management Program. You will support older adults and families by conducting assessments, developing individualized service plans, providing counseling, and connecting clients with benefits and resources to maintain independence at home. Schedule: Monday–Friday, 9:00 AM–5:00 PM; Hours per week: 35.

Requires MSW; LMSW preferred. Fluency in English and ability to travel across the community are important.

Qualifications

• Master of Social Work (MSW) or an equivalent master's-level degree in social work or related field. LMSW credential preferred but not required.
• Strong written and verbal communication skills, including clear case notes, service plans, and reports.
• Ability to work independently and manage multiple client needs with strong organizational skills.
• Computer proficiency and ability to use electronic case management and reporting systems.

Responsibilities

• Conduct comprehensive assessments in clients' homes and offices using NYC Aging data system as required.
• Develop and maintain written case plans tailored to each client's needs and goals.
• Provide counseling, guidance, and education to clients and families.
• Screen clients for eligibility for benefits and other assistance programs.
• Reassess needs and monitor services to ensure appropriate support is provided; maintain accurate records.
