Job summary

Mount Sinai Health System is seeking a part-time RN Case Manager in New York, NY, to oversee case management for a group of patients and work with an interdisciplinary team to improve outcomes.

Responsibilities include admission reviews, ongoing stay assessments, discharge planning, and thorough documentation in the Case Management system. A NYS RN license is required; CCM preferred; BLS is required.

Qualifications

• Bachelor's Degree in Nursing is required.
• Master's Degree is preferred.
• 3–5 years RN experience in an acute care setting.
• Homecare/long-term care or utilization review experience preferred.
• Current NYS RN license; CCM preferred; PRI within 1 year; BLS required.

Responsibilities

• Review new admissions to identify case management needs.
• Perform continued stay reviews to justify inpatient stays.
• Collaborate with physicians to expedite diagnostics and treatments.
• Document clinical reviews in the Case Management Documentation system.
• Assess patients' clinical, psychosocial, and functional status.
• Identify variances affecting length of stay and discharge planning.
• Conduct follow-ups on treatment delays and reporting results.
• Implement and coordinate activities to facilitate patient care.
