Job summary

Mount Sinai in New York is seeking an RN Case Manager for full-time weekday, weekend, and holiday coverage. You will assess patients, coordinate care, and collaborate with physicians and the care team to optimize LOS and reimbursement.

Responsibilities include utilization review, discharge planning, and documentation within the Case Management system. Requires NYS RN license; CCM and PRI within 1 year are preferred. Strong teamwork and communication are essential.

Qualifications

• Bachelor's Degree in Nursing; Master's degree preferred.
• Current NYS RN license required; CCM preferred; BLS certification (AHA) noted.
• PRI Certification to be obtained within 1 year; CCM re-credentialing expected.

Responsibilities

• Reviews new admissions to identify need for utilization review and discharge planning.
• Performs ongoing reviews to justify continued inpatient stay per criteria.
• Collaborates with physicians and clinicians to expedite testing and treatment.
• Documents all reviews in Case Management Documentation system.
• Develops and implements goals with physician, team, patient and family.
• Coordinates activities to move patient through continuum of care.
• Identifies readmission causes and works on reduction strategies.
• Interfaces with departments for discharge planning and authorization.
