Job summary

Mount Sinai Health System in New York, NY seeks an RN Case Manager to oversee case management for assigned patients, ensuring appropriate care levels and collaborating with an interdisciplinary team to improve outcomes.

You will review admissions, conduct continued stay reviews, and work with physicians to expedite testing and treatment, while documenting all clinical reviews in the system. Strong clinical and communication skills are required.

Qualifications

• Bachelor's Degree in Nursing required; Master's preferred.
• Current NYS RN license required; CCM preferred.
• PRI Certification to be obtained within 1 year.
• BLS certification required.
• 3–5 years RN acute care experience preferred.
• Experience in homecare/long-term care or utilization review preferred.

Responsibilities

• Review admissions to identify needs for utilization review and discharge planning.
• Perform continued stay reviews to justify inpatient stays.
• Collaborate with physicians to expedite diagnostic testing and treatment.
• Document all clinical reviews in the Case Management Documentation system.
• Assess patients' clinical, psychosocial, and functional status with the interdisciplinary team.
• Identify variances affecting length of stay and discharge planning.
• Conduct follow-ups on treatment delays and reporting of results.
• Develop specific goals with the physician and interdisciplinary team.
