Qualifications

• Bachelor's Degree in Nursing required.
• Master's degree preferred (not mandatory).
• 3–5 years RN experience in acute care recommended.
• CCM certification preferred; re-credentialing expected.

Responsibilities

• Reviews all new admissions to identify patients requiring utilization review, discharge planning, and/or case management to achieve optimal outcomes and appropriate reimbursement.
• Performs continued stay reviews using standardized criteria to justify inpatient stay.
• Collaborates with physicians and other clinicians to expedite testing, treatment and consultations.
• Documents all clinical reviews in Case Management Documentation system.
• Assesses discharge planning needs and coordinates with the team to ensure timely progression of care.
• Coordinates with financial and payer teams for authorization and post-discharge planning.
• Maintains liaison with interdisciplinary team to minimize delays and readmissions.
