Mount Sinai Queens is seeking a Care Manager-Care Management for a full-time day shift. You will coordinate patient care across the continuum, ensuring quality, clinical and cost outcomes, and collaborate with physicians, social services and nursing.

The role requires NY RN license with 3–5 years acute care and PRI certification. You’ll manage admissions, discharge planning, and care coordination activities to support safe and efficient patient care.

Qualifications

• RN licensed to practice in New York (state license).
• 3–5 years acute care experience with care management duties.
• PRI Certification.

Responsabilités

• Review admission/continuation charts for appropriateness.
• Coordinate communication across departments to complete tests and follow-up.
• Follow up on abnormalities and delays with documentation and treatment.
• Facilitate interdisciplinary rounds with physicians, social services and nursing.
• Refer cases to Risk Management when appropriate.
• Conduct concurrent and retrospective managed care reviews.
• Assess patient stability for Alternate Level of Care and obtain signatures.
• Enter ICD-9 and data entry codes for Care Management worksheets.
• Support discharge planning forms and processes with Social Services.
