Job summary

Advocate Health is seeking a part-time RN Case Manager to support case management and discharge planning across our North Carolina facilities. PRN schedule requires flexibility to cover 32 hours per shift, with preference for hospital market availability in Concord, Lake Norman, and University City.

Qualified candidates hold a BSN, RN license (or NP where applicable), and at least two years in healthcare with case management experience. Certification due within 3 years of hire; BLS is required.

Qualifications

• BSN required; Masters preferred.
• Current RN license or temporary RN Practitioner license; state licensure required where you work/reside or NP licensure per state rules.
• Two years health care experience with case management/discharge planning in acute, home, LTC, physician office, or managed care setting.
• Certification required within 3 years of hire per departmental protocol.
