Job summary

Moments Hospice is seeking a Part-Time Registered Nurse – Hospice to serve as the RN Case Manager in the field. You will conduct patient assessments, develop care plans, administer skilled nursing care, and educate families on end-of-life care. Responsibilities

• Complete initial and ongoing comprehensive assessments of patients' physical, emotional, and psychosocial needs
• Develop, implement, and update individualized plan of care in partnership with patients and families
• Provide skilled nursing care, including pain and symptom management, medication administration, and comfort measures
• Educate patients and caregivers on disease progression, symptom management, medications, and what to expect at end of life
• Coordinate care across your caseload and serve as the primary liaison between patients, families, physicians, and the care team
• Participate in interdisciplinary group (IDG) meetings to ensure comprehensive, holistic care

Schedule: Monday–Wednesday, 10:00 AM – 6:00 PM
