Job summary

Moments Hospice is seeking an experienced RN Case Manager to lead clinical care for patients and families in Minnesota. You will assess needs, coordinate services, and support symptom management in a field-based role with a Monday–Friday schedule. A company fleet car is provided to support patient visits.

The role requires a current RN license in the state, strong EMR skills, and a compassionate, patient-centered approach. Competitive salary and benefits are included.

Qualifications

• Active RN license in the state you work in (Minnesota).
• Proficiency with electronic medical records (EMR).
• Strong organizational and communication skills.
• Ability to think critically and work independently.
• Compassionate, patient- and family-centered approach.
• Valid driver's license and reliable vehicle with current insurance.

Responsibilities

• Complete initial and ongoing comprehensive assessments of patients' physical, emotional, and psychosocial needs.
• Develop, implement, and update individualized care plans with patients and families.
• Provide skilled nursing care, including pain and symptom management and medication administration.
• Educate patients and caregivers on disease progression, symptom management, and end-of-life expectations.
• Coordinate care across your caseload and act as primary liaison among patients, families, physicians, and the care team.
• Participate in interdisciplinary group (IDG) meetings to ensure holistic care.

Pay: $75,000 – $90,000 per year

Job Type: Full-time | Field-based

Schedule: Monday–Friday, 8:30 AM – 5:00 PM
