Moments Hospice is seeking a Weekend Registered Nurse to provide skilled, compassionate care in a hospice setting for Saturday-Sunday shifts. As the clinical lead for your patients and their families, you will assess needs, manage symptoms, and coordinate care while maintaining a high standard of comfort and dignity.

Qualifications

• Active RN license in the state of work.
• Proficiency with electronic medical records (EMR).

Responsibilities

• Complete initial and ongoing comprehensive assessments of patients' physical, emotional, and psychosocial needs.
• Develop, implement, and update individualized plan of care in partnership with patients and families.
• Provide skilled nursing care, including pain and symptom management, medication administration, and comfort measures.
• Educate patients and caregivers on disease progression, symptom management, medications, and end-of-life expectations.
• Coordinate care across caseload and serve as primary liaison between patients, families, physicians, and the care team.
• Participate in interdisciplinary group (IDG) meetings to ensure holistic care.

Pay: $75,000 – $85,000 per year

Job Type: Part-time | Field-based

Schedule: Saturday and Sunday, 8:00 AM – 8:00 PM
