Job summary

Moments Hospice is seeking an experienced RN Case Manager to lead patient care, coordinate services, and support families through end-of-life transitions. You will assess needs, manage symptoms, and collaborate with the care team in a field-based role across Minnesota.

This full-time position offers a competitive salary of $75,000–$90,000 per year, plus a fleet car program, comprehensive benefits, and opportunities to influence care delivery while reporting to leadership.

Qualifications

• Active Registered Nurse (RN) license in the state you work in.
• Proficiency with electronic medical records (EMR).
• Strong organizational, communication, and critical-thinking skills.
• Ability to work independently while maintaining high standards of care.
• Compassionate, patient- and family-centered approach.
• Valid driver's license and reliable vehicle with current insurance.

Responsibilities

• Complete initial and ongoing comprehensive assessments of patients' physical, emotional, and psychosocial needs.
• Develop, implement, and update individualized plan of care in partnership with patients and families.
• Provide skilled nursing care, including pain and symptom management, medication administration, and comfort measures.
• Educate patients and caregivers on disease progression, symptom management, medications, and what to expect at end of life.
• Coordinate care across your caseload and serve as the primary liaison between patients, families, physicians, and the care team.
• Participate in interdisciplinary group (IDG) meetings to ensure comprehensive, holistic care.

Pay: $75,000 – $90,000 per year

Job Type: Full-time | Field-based

Schedule: Monday–Friday, 8:30 AM – 5:00 PM
