Job summary

Moments Hospice is seeking an RN Case Manager in a field-based role in Illinois. You will lead clinical care, manage symptoms, coordinate services, and provide guidance to patients and families during end-of-life care.

This full-time position offers a fleet car program, comprehensive insurance, PTO, and a clear path for professional growth. Monday–Friday schedule with a stable caseload and collaboration with the care team.

Qualifications

• Active RN license in the state you work
• Proficiency with electronic medical records (EMR)
• Strong organizational, communication, and critical-thinking skills
• Ability to work independently while maintaining high standards of care

Responsibilities

• Complete initial and ongoing comprehensive assessments of patients' physical, emotional, and psychosocial needs
• Develop, implement, and update individualized plan of care in partnership with patients and families
• Provide skilled nursing care, including pain and symptom management, medication administration, and comfort measures
• Educate patients and caregivers on disease progression, symptom management, medications, and what to expect at end of life
• Coordinate care across your caseload and serve as the primary liaison between patients, families, physicians, and the care team
• Participate in interdisciplinary group (IDG) meetings to ensure comprehensive, holistic care

RN Case Manager – Hospice

Pay: $75,000 – $90,000 per year

Job Type: Full-time | Field-based

Schedule: Monday–Friday, 8:30 AM – 5:00 PM
