Job summary

Moments Hospice is seeking a Weekend Registered Nurse to lead clinical care for a dedicated caseload on Saturdays and Sundays, 8:00 AM – 8:00 PM. You will assess patients, coordinate plans of care with families, and deliver symptom control, education, and compassionate support in a hospice setting.

You will work in a collaborative team with leadership access and opportunities to shape your career while maintaining reasonable caseloads and clear expectations.

Qualifications

• Active RN license in the state of work.
• Proficiency with electronic medical records (EMR).
• Strong organizational and communication skills.
• Ability to work independently with high care standards.
• Compassionate, patient- and family-centered approach.
• Valid drivers license and reliable vehicle.

Responsibilities

• Complete initial and ongoing comprehensive assessments of patients' physical, emotional, and psychosocial needs
• Develop, implement, and update individualized plan of care in partnership with patients and families
• Provide skilled nursing care, including pain and symptom management, medication administration, and comfort measures
• Educate patients and caregivers on disease progression, symptom management, medications, and what to expect at end of life
• Coordinate care across your caseload and serve as the primary liaison between patients, families, physicians, and the care team
• Participate in interdisciplinary group (IDG) meetings to ensure comprehensive, holistic care
