Job summary

Enhance Rehabilitation in Milwaukee, WI is seeking qualified Physical Therapists to join our dynamic team in SNF settings. You will evaluate and treat patients following an individualized plan of care, documenting progress and adjusting goals.

Responsibilities include supervising Physical Therapist Assistants, collaborating with facility staff, providing patient education on injuries and outcomes, and delivering excellent care and customer service.

Qualifications

• Qualified Physical Therapists will evaluate and treat patients per individualized plan of care.
• Provide high quality patient care and documentation.
• Supervise Physical Therapist Assistants as needed.

• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
